I don't have lean PCOS but I've also struggled my whole life with terrible migraines (pulsing/throbbing pain, jaw pain, vomiting etc) that are also likely linked to PCOS. Good luck, glad you are seeing progress, keep going! It may be helpful to keep a log re: food, sleep and see if you can pinpoint any of your additional migraine triggers. Managing cortisol - sleep and stress - is also important, it also increases IR. Supplements can help with insulin sensitivity but to reduce the amount of insulin the keys are what you eat, how often and how early. One other thing to experiment with might be some time restricted eating or intermittent fasting, it was very impactful for my IR. Are you taking 4g per day of the inositol? Can't remember which brand recommends 2 but 4 is the standard rec for PCOS. If you look in r/ketoscience or on keto subs there are a lot of people who have had headache improvement. It might be helpful to experiment with lowering dietary starch more, some "gluten free" processed food products are super high in carbs and some people are affected by sweeteners, not sure what you are eating, but throwing that out. Lowering insulin often helps, I have not had a migraine since cutting sugar and starch. These sites have useful articles, though be aware that they may also be trying to sell you something.
